Just got this from a contact in The Bills locker room. It's a locker tag from 2006 for safety Matt Bowen.

Thats something you don't see very often (at all really), is it vinyl or a plaque? Nice logo above it too, is that custom made?

my wife got this made for my birthday recently, not strictly a collectable though Smile

Awesome cake!! happy Birthday recently. The name plate is plastic lamanate. The Logo above is made from wood and it's custom made afro

i work in a sign shop and would be able to make that sort of label for you if you ever needed it, as well as any of the usual sign stuff (stickers/banners etc) Just a thought
